この申請により発行されるのは、Web問診を受検したことの証明です。
健康診断の検査項目に関する証明はできませんので、
お申込み間違えのないようにしてください。

• 健康診断受検証明書は、Web問診入力を完了（登録）した翌月以降に、
  申請に基づき発行します。
  発行は、申請から3日以降後になります。
  例1：

  5月10日	Web問診入力
  5月10日	健康診断受検証明書　発行申請
  6月1日以降発行可能

  例2：

  4月10日	Web問診入力
  7月10日	健康診断受検証明書　発行申請
  7月13日以降発行可能

What will be issued by this application is the proof that you have taken the web-based medical questionnaire.
Please make sure you have applied correctly.

• A certificate of health checkup will be issued based on your application after the month following the month in which you complete (register for) the web medical questionnaire.
  The certificate will be issued 3 days or more after the application is submitted.
