健康診断書発行に関する問い合わせ FAQ

Q1　学生です。就職活動で診断書が必要です。学生定期健康診断を受けていないと診断書が作れないと聞きましたが、本当ですか？

A　本当です。学生定期健康診断を受けていない方は作成できません。
診断書作成を受け付けている他の医療機関に各自電話でお問い合わせ下さい。

 

Q1. I am a student. I need a Medical Certificate for job-hunting. I heard that if I did not take the student health check, I cannot get one. Is this true?

  1. It is true. If you did not undergo the student health check, we cannot make a Medical Certificate for you. Please call and consult with a different medical facility that can make a Medical Certificate.

Q2　実際にどんな診断書が作れますか？

A　就職用、留学用、海外渡航用、実習用、医療系資格取得用（医師・看護師・薬剤師・臨床検査技師等等です。指定の用紙がある場合は各自でご持参ください。

 

Q2. Exactly what kind of Medical Certificate can you make?

  1. We can make Medical Certificates for job-hunting, studying abroad, traveling abroad, self-study, receiving medical qualifications (doctor, nurse, pharmacist, clinical technician, etc.), and so on. If there is a specific form you need filled, please bring it with you.

Q3　英文や中国語の診断書は作成できますか？

A　各自で翻訳していただき、必要な検査項目・記入事項をご説明いただけましたら可能です。

 

Q3. Can you make an English or Chinese Medical Certificate for me?

  1. If you explain which tests and descriptions need to be translated, we can.

Q4　学生定期健康診断で後日尿検査を行いましたが、この結果は自動発行機から出せる結果通知書や証明書に反映されますか？

A　はい、後日尿検査の結果も反映されます。

 

Q4. I submitted my urine sample after the student health check period. Will the test results appear on my Health Checkup Results?

  1. Yes, urine samples submitted at a later date will also appear on Health Check Results.

Q5　他の医療機関で受けた検査の結果をもとに診断書を出してもらえますか？

A　できません。本学で受検した定期健診の結果や本健康管理室で受けた検査の結果のみ、診断書に引用することが可能です。ただし、引用できる検査日が規定されている場合がありますので、診断書の提出先にご確認ください。

 

Q5. Can you make a Medical Certificate from the test results of another medical facility?

  1. We cannot. We can only make a Medical Certificate from annual health check results and tests performed at a Kyoto University Health Care Office. There may be regulations for the test result dates, so please check the submission guidelines.
