学生、職員の健康診断結果（健康診断成績票）の発行、再発行を行います。

紙の学生証などで、証明書自動発行機で証明書を出力することができない学生も、健康管理室にて、健康診断結果通知書を発行します。

 

【学生】

健康管理室に取りに来てください

1. 健康診断結果通知書発行申請書をダウンロードして記入する。
2. 1の申請書と学生証　を持って健康管理室開室時間中に取りに来る。

 

【職員】

健康管理室に取りに来る場合

1. 職員健康診断成績票再発行申請書をダウンロードして記入する。
2. 1の申請書と職員証　を持って健康管理室開室時間中に取りに来る。

学内便で送付を希望する場合

1. 職員健康診断成績票再発行申請書をダウンロードして記入する。
2. ①1の申請書に記入したもの（PDF or WORD文書）
   ②職員証の写し（PDF or 画像）
   ①、②を添付して、uk_honbu*mail2.adm.kyoto-u.ac.jp(*を@に読み替えてください)宛てに
   メールを送ってください。

The Health Care Office will issue or reissue the health checkup results for students and employees.

Students with paper student ID cards, etc., who are unable to print out certificates from the automatic certificate issuing machine, will also have their health checkup results issued at the Health Care Office.

1. Download and fill in the application form for reissue of the health checkup result.
2. Come to the Health Care Office during the opening hours with the application form and your student ID card.

If you wish to have the form sent to you by the campus mail service

1. Download and fill in the application form for reissue of the health checkup result.
2. (1) Completed application form (PDF or WORD document)
   (2) A copy of your staff ID card (PDF or image)
   Send an email to uk_honbu*mail2.adm.kyoto-u.ac.jp (replace * with @) with (1) and (2) attached.
