令和8年度 学生一般定期健康診断は、4月2日(木)から4月23日(木)の間に行います。
健康診断は、健康管理、病気の早期発見に繋がります。
未受検の場合、所定の診断書等の発行ができないこともありますので、必ず受検してください。
2026 Health checkups for students will be held from April 2 to 23.
Checkups are important for your health management and can also help detect potential problems early. If you do not undergo an
examination, we may not be able to issue the prescribed certificates, etc.

Web 問診入力
Web questionnaire for students
学内ネットワーク※から問診登録WEBサイトにアクセスし、問診票への入力を行なってください。
※Web問診には学内ネットワーク（KUINS)および学内無線LAN（KUINS-Air）に接続されたPC・スマートフォン等からのみアクセス可能となっております。設定の詳細につきましては、情報環境機構のウェブサイトをご参照下さい。

The medical questionnaire needs to be filled out online by 24:00 on the day before the checkup date by KUINS or KUINS-Air. (available on April 3rd)
If you cannot successfully access the site or answer the questionnaire, you can use the on-site tablet to answer the questionnaire at the venue. On that case, you may have to wait for some time due to the limited number of the tablets.

Web問診登録方法 【令和8年4月3日(金)～受検前日まで】
- Web問診ログイン（学内限定）ボタンをクリックし、ログイン画面でECS-IDおよびパスワードを入力して下さい。
- Web問診マニュアル（PDF ）に従って操作してください。
★受検当日の入力は無効です
受検当日に未回答に気づいた場合は、健診会場のPCで回答してください
How to answer the Web questionnaire （From Apr. 3rd , by 24:00 on the day before the checkup date )
*If you choose not to take the medical checkup and only take the Web-based medical questionnaire, you can take the Web-based medical questionnaire until the end of November.
NOTICE: All incoming students (under graduates / graduates) MUST register the questionnaire on “New student’s site”.
- Click the Web Questionnaire site login (The university LAN limited access) and input your ECS-ID and passwords.
- Check the manual for information on how to answer the Web questionnaire.
★Input on the day of the examination is voided.
If you notice you have not answered the questionnaire on the day of the examination, please answer it on the PC at the examination venue.

健康診断の結果について About Results
健康診断の結果は『健康診断結果通知書』として証明書自動発行機で発行することができますので、発行開始日（6月1日）以降に各自で発行してください。詳細は以下の文書をご確認ください。
Medical examination results can be issued from certificate-issuing machines, so please issue the results by yourself after the start date of issuance (June 1). For details, please refer to the following document.
