この健診は、5月～8月にかけて健康管理室で実施するものです。

１． 対象者 Eligibility

教職員（※）のうち主として以下に該当する方を対象として実施します

　　　　　　 障害等による困りごとへの配慮（合理的配慮）が必要な者、
　　　　　　 計画された長期出張等により従来型健診の受検機会が得られない者

※但し、次に該当する者は除く。
・新たに採用された者で雇入時健康診断の対象となっている者
・本学学生の身分をあわせ持つ者（ＯＡ、ＴＡ、ＲＡ等）
・教職員に準ずる者（研修員、日本学術振興会特別研究員（PD）等）
・派遣職員

The reservation-type health check is mainly for the following Kyoto University faculty and staff members*:

• Persons who need special consideration (reasonable accommodation) due to disabilities, etc.
• Persons who will be unable receive the regular health check for faculty and staff due to business trips,
  etc.

*Those who fall under any of the following categories are not eligible:

• New employees who have received (or will receive) the health check for new employees.
• Persons who are also enrolled as students at the university (OAs, TAs, RAs, etc.).
• Persons who are “deemed equivalent to” faculty/staff members, but are not actual faculty/staff members
  (trainees, JSPS postdoctoral fellows [PDs], etc.).
• Dispatched staff

 

２．実施人数

実施期間を通じて３６０人（４０人/日）を予定しています。予定数に達し次第、受付終了となりますのでご了承願います。

４．実施場所　Location

本部構内　健康管理室１階　Health Care Office（Yoshida）

 

５．受検日時の予約方法　Resarvation

・ 下記『予約型健診Web予約』で予約手続きを行います。

昨年度、既にユーザー登録を行った人も、もう一度初回ユーザー登録を行う必要があります。

雇入れ時健診対象者は雇入れ時健診を受検ください。

初回ユーザー登録にて、パスワードの設定を行い、メモ等に記録するなどして各自で管理してください。

ログインに用いるIDは、SPS-ID／KING-IDではなく、職員証に記載されている「8桁の職員番号（左から8桁）」となりますので、ご注意ください。（職員番号はこちら）

・予約期間： 2023年5月9日(火)～7月28日（金）
　※実施日7日前まで（例外的に８月実施分の予約締切は最終が７月２8日（金）となります）

• The available dates and times for the health check will be shown on the online reservation website,
  please make a reservation between May 9 (Tue.) and July 28 (Fri.).
• Reserve your desired examination day online, at least 7 days in advance (exception: all
  August appointments must be made by July 28).
• Those without a reservation cannot receive the examination.
• The online reservation system can only be accessed through a computer or smart phone connected
  to the Kyoto University Network (KUINS, KUINS-Air). You cannot access it from outside of the
  campus.

※法定項目外（オプション）検査については、産業厚生部門への健康情報取得（提供）についての本人の同意がある場合のみ実施します。
※下記ボタンより、Web予約を行った場合には、健康情報取得（提供）について同意したものとみなします。
